Below, you will see two links to open both the application form and the waiver/release form. Please be sure to fill out both of these forms fully, and send them along with a post dated cheque - two weeks prior to the course start date! Cheques are not deposited until two weeks prior to the course, regardless of the date. (easier book keeping for hubby!)
If you have made an on line payment. Please print the waiver/release and the application form and mail or scan them to me. We do not have a fax. .

Please be sure to print both forms, and fill them out fully. If you do not have a witness for the waiver, then you will need to fill out the form again at the course (no problem!) Please be sure to have your witness actually witness your signature, as the dates on the waiver must match please. Thank you!
